Sodium dichloroacetate (DCA) is a small, orally bioavailable compound that functions as an inhibitor of mitochondrial pyruvate dehydrogenase kinase (PDK). It has been utilized in clinical settings for conditions such as MELAS syndrome and congenital lactic acidosis in children. Recent research suggests that DCA may also serve as a vasoprotective agent by inhibiting PDK2, which in turn reduces the proliferation of coronary endarterium. Additionally, DCA has shown promise in promoting brain regeneration following cerebral ischemia, indicating its potential significance in vascular diseases.
As a PDK inhibitor, DCA shifts pyruvate metabolism away from glycolysis and lactate production, favoring glucose oxidation within the mitochondria. This mechanism not only induces apoptosis in a mitochondrial-dependent manner but also reverses the downregulation of K V 1.5 channels specifically in cancer cells, while sparing normal cells. Furthermore, DCA has demonstrated the ability to decrease tumor growth in both in vitro and in vivo studies, highlighting its potent anti-leukemic properties.
